Information for Referrers

At Move to Live we use movement and exercise to help people with chronic and complex conditions, diseases, injuries, disabilities, and those who need some guidance and support when it comes to movement. We help to facilitate movement and physical activity when this is difficult to fit into your clients’ life, and work hard to provide a safe(r) space for people who have had unpleasant experiences in the health and fitness industry.

Each new client must undertake an Initial Assessment so a movement program can be tailored to their specific wants and needs. We ensure our clients are able to move their bodies safely and confidently during their sessions, and we encourage collaboration and referrals to other Allied Health Professionals to provide a well-rounded approach for our clients’ ongoing wellbeing.

    Referrers are always welcome to visit Move to Live for a tour, though we ask you to book with us in advance so we are able to accommodate you into our client schedule.

  • Move to Live offers a variety of services to help support our clients on their movement journeys.

    Everyone starts off with an Initial Assessment with one of our Exercise Physiologists where we collaboratively decide which of our services, or a combination thereof, will be suited for you.

    These include Individual Movement Sessions, Group Classes, Home Exercise Programs, Solo Time, Hydrotherapy, Home Visits, and Telehealth Services.

  • Move to Live provides support and guidance for those experiencing challenges with movement and physical activity. Our Exercise Physiologists tailor their support to suit your patients’ needs, including:

    • Using movement to improve their functional capacity with activities such as walking, transfers, stairs, and other difficult tasks.

    • Discovering how to move their body in ways which work with their disability and accessibility requirements.

    • Using movement to support and manage their health conditions and any co-occurring challenges that they may be experiencing.

    • Assisting with returning to work / sport / daily activities after injury through rehabilitation.

    • Understanding more about their health circumstances and how movement can help them.

    • Improving their relationship with movement and physical activity.

    • Learning how to integrate movement safely and confidently into their life.

    • Or … helping them when they are just not sure where to start.

    We are here to support your patients in whatever way they need and to meet them wherever they are at with movement and their health.
